HEALTH DECLARATION
By taking part in a Leisure Leagues small-sided football league, every player and staff member is agreeing to make a COVID-19 Health Declaration to confirm that:
You do not have any COVID-19 symptoms.
You have not received a positive test in the last seven days.
That no member of your household has Covid-19 symptoms or is waiting on a test result.
And that to your best of your knowledge you have not been in close contact with anyone who is exhibiting corona virus symptoms or has tested positive.
All players and staff members are also expected to maintain social distancing where possible and to keep themselves, fellow players and Leisure Leagues staff safe by adhering to the safety and guidance given.
Every time you take part in a Leisure Leagues provided sports league you will effectively be making the above Health Declaration.
